We took over an account that has massive amounts of claims that are no claim on file and past timely for both gov and private insurance. In the past screen shots proving we sent the claim would help at least some of these claims but now most insurance companies won't even accept that, any ideas what other forms of proof we could use? Oh and there is no clearinghouse for the majority of these claims.

Hi,

- Prepare the list of Insurances for which you have the denied claims
- For most of commercial payers, they have their separate forms for reconsideration. For example UHC has reconsideration form on their website. If any payer don't have any specific form, prepare your format and file an appeal.
- Attach the submission proof which can be systematic report and if not possible prepare yourself a format which contains submission date, time, claim charges and other details which could stand as proof of timely filing
- Do it ASAP because most of the payers have appeal filing limit starting from the date of denial which is too limited (i.e. 60, 90 days).

As soon as you will file reconsideration forms, it will increase the changes to get them paid.
