I do HCC coding. I'm not certified, but have been doing HCC coding for years. This has always bugged me. Why when a patient has a diagnosis code of 250.50, is it always coded with 362.01, if there is no diagnosis linking the 250.50?

Joe has 250.50. So, with our system, I would code Joe with 250.50 and 362.01. How do you know know he has 362.01? is that just the rule? If he had renal manifestations, how do you know he has 583.81 (which doesn't map to an HCC)


Same with 250.60. How do I know he has 357.2? It's eating at me,and I decided to ask. I know what the ICD-9 coding book says to do. But how do you know for sure?
