Does Medicare require UB92 claim forms/format for a surgical facility (EGD, Colonoscopy, Flexsig, etc)  if so, how does it differ from billing on a CMS 1500 format.  thank you!

The UB 92 no longer exists.  it was replaced by the UB 04. The UB form is for billing facility fees and inpatient services. The CMS 1500 form is for outpatient and provider services.   The difference between the UB 04 and CMS 1500 form is like a bicycle and an automobile.

To answer your question, outpatient surgical facilities are billed on HCFA 1500s.  If it is in fact a surgical hospital, with inpatient beds, the UB-04 is required to submit the facility charges.  If its your first time to bill on UBs check out a resource like a UB Editor which walks you through what goes in each blank.
