Per Medicare this is unbundalbe with the use of a Modifier and they will pay it.  Does anyone know what that Modifier would be?

I am in an urgent care setting.  We see this all the time.  We append the modifier 25 to the office visit.

I am having the same problem with Palmetto since 1/1/09.  Denial reason is "service not payable with other service rendered on the same date".  CPT 96372 is being denied even with modifier -25 after EM code.

I had the same problem, and finally, after a month of finding no information about the problem, Palmetto GBA (our carrier) has issued an Alert that they've "made an error and a mass adjustment will be taking place".   I don't know if it's because I sent a complaint to their own site's link to cms.gov re: 'how we're doing' or not, but at least they've acknowledged the problem.
