If patient is admitted for osteoarthritis of the knee and replacement, and it says arthritis in the past medical history, do you code just the osteoarthritis as it's more specific or do you code both?

Code Dx only on the currently treated problem. I never code dx from past medical history.

Sometimes they list things like hypertension, hypercholesterol, etc.  that they have and are being treated for.  On this particular H&P, it is the only place they mentioned hypertension.  So that's why we code from what they have listed under the past medical history.  

If they had mentioned " arthritis" under say review of systems, would you code it or just the osteoarthritis?
