Patient came in and was treated because of acute delirium.  I querried the physician for a cause of the delirium and he said hyponatremia/malignant hypertension.  Do I code the acute deliruim as principal or the hyponatremia/malignant hypertension?

IP or OP?  Either way the Dr. needs to document the query answer in the record and state it as such that the delirium was caused by XXX or XXX.  Then follow the rules for the place of treatment- in the hospital- could code either following the "That which after study caused admission" rule; OP is always a different story- maybe an OP coder can help there.

The patient was an acute patient.  I'm coding the hyponatremia first. Hope that's right.
