If discharge summary says "colon mass most likely cancer, do you code the colon mass or the colon cancer?  Biopsies are pending at time of discharge.  This is an acute patient.

You would code the mass, because at the time of discharge a cancer dx has not been confirmed.

That's what I thought but I guess I questioned it because you can
code "possible" diagnoses on inpts.

You can always code suspect, probable and possible.  Use the code that ends with .89 and use modifier 59 with the CPT
