The company Mag Mutual produces both a hard copy and an electronic version of fees paid nationally.  They give the Medicare national average as well as the high/low range for commercial payers.   My contact at Mag Mutual is Jennifer Staples -- jstaples@magmutual.com or 1.678.226.0044.  The hard copy is a fabulous resource for anyone dealing with coding and reimbursement issues in that it gives the background of any reimbursement issues, and tells you if the CMS payment policy is different from the commercial payer policies in their experience.  I almost sleep with mine.  That manual is called the 2006 Physicans' Fee and Coding Guide.   They electronic versions are called Fees On Disk (Standard and Professional levels).
