I’m working on having a better understanding of CPT coding. I’m requesting advice on the following example:

I’m employed by an inpatient psychiatric hospital.  We use our provider services codes for billing and to track productivity.

I have an APN admitting the patient.  The APN provides the Initial Hospital Care (99221-99223) service and also completes the Initial Psychiatric (90791-90792) service on the same day.  Realizing that I cannot bill both services under the same provider on the same date of service, would it be better to bill for the Initial Hospital Care (99221-99223) service and use the Initial Psychiatric (90791-90792) service entry for productivity tracking only?  Or…should I bill for the Initial Psychiatric (90791-90792) service and use the Initial Hospital Care (99221-99223) service for productivity tracking?

The way our billing system works, an error report is created if there is no CPT code entered for the Initial Hospital Care (99221-99223) service, the Initial Psychiatric (90791-90792) service, and the Discharge (99238-99239) service on each patient episode.  So that is why I need to submit a code from each group of CPT codes.

I hope this makes sense.
